YOUR MEMBERSHIP IS PENDING. We will hold onto your enrollment form for 30 days. You need to verify your identity by coming to a Jacks event in person and presenting your photo ID. After 30 days, if you haven’t come to the Jacks, all your enrollment data, membership agreements and liability waiver will be deleted.
